This work is an examination of how safety-related data is currently being managed across the healthcare domain in the UK. Some areas where safety data can cause specific problems are highlighted and extant mitigations noted. Some case studies where data has been a contributory factor in the occurrence of harm are discussed. The work concludes with new proposed guidance material to supplement the existing safety guidance on Health IT Systems.
