Families of children left disabled by an epilepsy drug and women injured by pelvic mesh implants should be given urgent financial help, England's patient safety commissioner has said.

The chief executives of Post Office and Fujitsu are set to be questioned by MPs over the Horizon scandal.
The Horizon scandal saw more than 700 subpostmasters and subpostmistresses handed criminal convictions after faulty Fujitsu accounting software made it appear as though money was missing at their branches. Victims have since described being shunned by their communities, financially ruined and having their families destroyed.

Alaska Airlines says it has returned some of its grounded Boeing 737 Max 9 jets to service after one of its planes lost a section of fuselage mid-flight.
The airline apologised to those affected, saying an inspection found "no concerning findings". The plane had to make an emergency landing in the US state of Oregon after an outer section of the plane fell off.

“Urgent action is needed to address NHS computer failings which are causing harm to patients”, the patient safety watchdog has said. The watchdog has evidence of patient deaths due to IT system errors. The government called the reports "concerning" and said it would work with NHS England to take necessary action to protect patients.

A coastguard aircraft involved in a fatal collision at Tokyo's Haneda airport was not cleared for take-off, Japanese authorities have said. The smaller aeroplane collided with a Japan Airlines (JAL) passenger jet on the runway on Tuesday.
Released transcripts of air traffic control instructions just before the crash confirmed the JAL Airbus A350 was cleared to land.
Permission for take-off was not given to the coastguard Bombardier Dash-8.
